Agricultural machines manufacturer Superior Machines Ltd was fined tens of thousands of pounds for safety breaches, after a 68-year-old worker got entangled in a machine which resulted in his left arm being amputated.
He suffered other serious injuries such as a punctured lung, broken neck, fractures to his back, ribs, arm and shoulder blade.
An investigation by the Health and Safety Executive found machinery standards across the site were poor, with many machines unguarded. Superior Machines Ltd were served three prohibition and five improvement notices.
Superior Machines Ltd pleaded guilty to breaching section 2(1) of the Health & Safety at Work Act 1974 and the company was fined £60,000.
These were life-changing injuries that could have been avoided provided Superior Machines performed basic safeguards, risk assessments, supervision and if instructions had been in place.
